Referral Process
Choose from four convenient ways to submit referrals:
Use our therapy-specific forms.
Complete the form on the right to initiate a referral. Our intake team will contact you shortly after you submit the form.
Call us at 833-285-5500.
Fax your referral to 985-792-9004.
The Coastal Infusion Service team will:
- Verify benefits with patient’s insurance company and coordinate care. We handle collaboration between the patient and referring office.
- Schedule service. We will contact the referring office if authorization is needed.
- Provide follow-up services. We will update the referring office on patient status.
